Atrial Septal Defect (ASD) Repair

An atrial septal defect (ASD) is a hole in the atrial septum leading to left-to-right shunting and volume overload of the right atrium and right ventricle.

Children are rarely symptomatic but long-term complications after age 20 include pulmonary hypertension, heart failure and arrhythmias.

Adults and less commonly adolescents may suffer from exercise intolerance, shortness of breath, fatigue and arrhythmias. The diagnosis is established by echocardiography. Treatment consists of the use of a transcatheter closure device or surgery.

    Key Points :
  • An atrial septal defect (ASD) is a hole in one of the several parts of the atrial septum, leading to shunting of blood from left to right.
  • Small atrial messages often close spontaneously, but larger ones do not, causing right atrial and ventricular overload and ultimately pulmonary artery hypertension, increased pulmonary vascular resistance, and right ventricular hypertrophy; SVT, atrial flutter, or atrial fibrillation may occur.
  • ASDs can allow vein emboli to enter the systemic circulation (paradoxical embolization), leading to arterial occlusion (eg, stroke).
  • Moderately large ASDs should be closed, usually by age 2–6 years, using a transcatheter device if possible.
